Dear Patients,
Firstly, I would like to thank everyone - our team as well as our patients for your understanding, patience and cooperation during these past few months.
At this time, I would like to highlight some of the changes we have made at Dr. D.L. Sam Inc. Our priority is the health and safety of all our patients and our dedicated dental team. Please help us keep everyone safe during these uncertain times by abiding by our office policies and procedures during the COVID-19 pandemic. We appreciate your commitment to keeping the community safe!
Some of the changes include the following:
All individuals entering the clinic must wear a mask. (Exceptions will be made for young children and individuals unable to wear a mask for medical reasons.)
Patients will be required to complete a COVID-19 Patient Pre-Screening form as well as a Risk Acknowledgement Form prior to their appointment.
Patients will be pre-screened prior to their appointment and again upon entrance to the office.
Temperature checks will be taken upon entering the practice.
Hand sanitizer will be provided upon entrance that patients will be asked to use.
Barriers have been placed in the reception area to protect patients and dental team members.
For physical distancing we no longer have a physical waiting room.
Patients must check in virtually by calling the front desk at 604-437-1234 or responding to our day of appointment reminder text.
Only individuals being treated are allowed to enter the clinic. Accompanying persons are not permitted to enter, with the exception of caregivers (ie. very young children or individuals requiring an advocate may have one person accompany them to their appointment).
We are only accepting payment by Credit or Debit. No cash transactions at this time.
To minimize interaction we are avoiding all cash transactions at this time, therefore we will no longer be reimbursing parking fees.
The number of patients at any given time in the practice will be limited and appointments will be staggered.
Common areas will be cleaned and sanitized throughout the day.
Appropriate personal protective equipment (PPE) will be worn by all clinical and administrative staff.
Staff will be screen for COVID-19 symptoms on a daily basis
Treatment rooms will be completely sterilized before patients are seated - extra time will be scheduled in-between appointments to allow for the thorough cleaning and disinfection of treatment rooms and equipment.
